Objective: To investigate the efficacy of Ulinastatin for the patients with chronic heart failure, and to explore the impact on cell factor and cardiac function. Methods: All 60 patients with chronic heart failure patients hospitalized were randomly divided into the study group and the control group, with 30 cases in each group, and the two groups were both given conventional treatment and the study group was given Ulinastatin 400 000 LJ+50 ml GS liquid micro-pump on the basis of conventional therapy 1 h input, 2 times/d, for 5 d, and the follow projects were observed and compared: ①The efficacy of the two groups; ②the changes of cardiac function before and after treatment by echocardiography; ③the changes of IL-6, IL-8, BNP before and after treatment; ④security situation. Results: ①The efficiency of the study group was significantly higher than the control group (P<0.05). ②After treatment, LVDD, LVWP were significantly lower than before treatment (P<0.05), and LVEF was significantly higher than before treatment (P<0.05); and after treatment, the LVDD, LVWP, LVEF of the study group were improved more significantly than the control group (P<0.05). ③After treatment, IL-6, IL-8, BNP of two groups were significantly decreased compared with before treatment (P<0.05). And after treatment, IL-6, IL-8, BNP of study group were decreased more significantly than the control group (P<0.05). ④There were no significant adverse reactions between two groups during treatment. Conclusion: Ulinastatin is effective for the chronic heart failure, and it can significantly improve cardiac function, and through further analysis, it may be due to its lower pro-inflammatory cytokines by IL-6, IL-8, BNP levels and further improve cardiac function.%目的:探讨乌司他丁对慢性心力衰竭患者的疗效及对细胞因子与心功能的影响.方法:将60例慢性心力衰竭患者按照入院先后顺序随机分为研究组和对照组,各30例,两组患者均给予心衰的常规治疗,研究组在常规治疗基础上联合应用乌司他丁40万U+50 ml液体,微量泵1 h输入,2次/d,连用5 d.观察比较,①两组的疗效;②两组治疗前后超声心动图检查各心功能指标变化情况;③两组治疗前后IL-6、IL-8、BNP变化情况;④安全性情况.结果:①研究组总有效率明显高于对照组(P＜0.05).②两组治疗后LVDD、LVWP分别与治疗前比较均明显降低(P＜0.05),而LVEF明显升高(P＜0.05);且治疗后两组间LVDD、LVWP、LVEF比较,研究组较对照组改善更明显(P＜0.05).③两组患者治疗后IL-6、IL-8、BNP均较治疗前明显下降(P＜0.05),且治疗后研究组IL-6、IL-8、BNP较对照组下降更明显(P＜0.05).④两组治疗期间均未见明显不良反应发生.结论:乌司他丁治疗慢性心力衰竭疗效确切,能明显改善患者的心功能,分析可能是由于其通过下调促炎因子IL-6、IL-8、BNP的水平而进一步改善心功能.
