LISS技术治疗89例跟骨骨折的疗效分析

         

摘要

目的:探讨LISS技术治疗跟骨骨折的应用及临床疗效。方法选取2013年5月~2015年1月我院收治的178例跟骨骨折患者作为研究对象,随机分为观察组和对照组,每组89例,观察组采用LISS技术治疗,术前常规根据跟骨X 线片及CT进行分型。对照组患者采用DHS进行治疗;随访时间6~25个月,平均(15.8±1.2)个月。术后观察切口愈合情况,跟骨后关节面复位情况,跟骨的高度及宽度复位情况。采用美国骨科足踝外科协会(AOFAS)踝-后足评分标准对手术前、后足部功能进行评估。结果观察组患者均未发生感染,无一例发生钢板外露,所有切口均完全愈合,且无一例患者需二次手术来达到切口的愈合,治疗总有效率为100%,明显优于对照组的87.64%。且术后观察组随访时的跟骨宽度、高度、Böhler角、Gissane角及跟骨后关节面移位与术前比较均明显优于对照组,差异有统计学意义(P<0.05)。参照美国足踝关节评分标准(AOFAS)评分,优51足,良37足,中15侧,综合优良率达85.4%。结论 LISS技术适用于大多数的跟骨骨折,是一种安全、实用的治疗方法。%Objective To study the application and clinical effect of LISS technology in the treatment of calcaneal fracture. Methods 178 patients with calcaneal fracture, who were recevied and treated in our hospital from May 2013 to January 2015, were selected as the research objects and were randomly divided into observation group and control group, with 89 cases in each group. The observation group was treated with LISS technology, was separated routine type according to bone x-ray films and CT, while the control group was treated with DHS technology, to follow-up 6-25 months with the average of (15.8±1.2) months. To observe the incision healing condition, the reduction condition of facies articularis talaris posterior, the reduction condition of calcaneus height and width after operation. To evaluate the foot function of pre and post operation according to ankle-metapodium marking standard of America Orthopaedics Foot and Ankle Society (AOFAS).Results The treatment total effective rate in observation(100%) without infection occurrent, without plate exposure occurred in one case, with all incision complete healing without the second operation, was obviously better than which in control group(87.64%). And the calcaneus width, height, Böhler angle, Gissane angle, and the displacement of facies articularis talaris posterior in observation group after operation were obviously better than which before operation, and better than which in control group, the differences were statically significant(P<0.05). Referring to AOFAS score, the compositive excellent and good rate was 85.4% with optimal 51 feet, good 37 feet, medium 15 feet.ConclusionLISS technology is appropriate to most calcaneal fracture, is a safety and practical treatment methods.
