Objective To observe the efficacy and side effects of lobaplatin with fluorouracil (5-FU) in the treatment of advanced esophageal carcinoma. Methods Forty-eight advanced esophageal carcinoma patients divided into observing group ( n = 26) and control group ( n =22) were enrolled in this study from August 2009 to March 2010. The observing group was applied with lobaplatin (30mg/m2 d,) plus 5-FU(500mg/m2 d,-d, ) regimen. The control group was treated with cisplatin (20mg/m2 d,-d4) plus 5-FU (500mg/m2 d, -d, ) regimen. Twenty-one days was a cycle in two groups. Results Efficacy and toxicity could be assessed in all the 48 patients. The effective rate in observing group and control group were 53. 8% ( 14/26) and 50. 0% (11/22) , respectively. The main toxicity was digestive reaction and myelosuppression. The incidence rate of nausea and vomiting in observing group was lower than that in control group (P < 0. 05 ) , while the incidence rate of thrombocytopenia was higher than that in control group ( P < 0. 05). The median time to progress in observing and control group was 3. 7 and 3. 4 months, and median overall survival was 8. 7 and 8. 2 months. Conclusion Lobaplatin combined with 5-FU was effective and safe for advanced esophageal carcinoma, worth further study.%目的 观察洛铂联合氟尿嘧啶(5-FU)治疗晚期食管癌的疗效和毒副作用.方法2009年8月至2010年3月,将48例晚期食管癌患者分为观察组(n=26)和对照组(n=22).观察组:洛铂30mg/m2静滴,d1；亚叶酸钙200mg/m2静滴,d1～d5；5-FU 500mg/m2静滴,d1 ～d5.对照组:顺铂20mg/m2静滴,d1～d4；亚叶酸钙200mg/m2静滴,d1～d5；5-FU 500mg/m2静滴,d1～d5.两组化疗均21天为l周期.比较两组的疗效、不良反应以及生存随访情况.结果48例患者均可评价疗效,其中观察组的有效率为53.8%,对照组为50.0%,两组差异无统计学意义(P＞0.05).两组主要不良反应为消化道反应和骨髓抑制,其中观察组的恶心呕吐发生率低于对照组(P＜0.05),血小板减少发生率高于对照组(P＜0.05).观察组和对照组的中位疾病进展时间分别为3.7个月和3.4个月,中位生存期分别为8.7个月和8.2个月.结论洛铂联合氟尿嘧啶治疗晚期食管癌的疗效较好,毒副反应可以耐受,值得临床进一步研究应用.
