目的：探讨经尿道前列腺等离子电汽化切除术联合抗雄激素治疗晚期前列腺癌的临床疗效。方法：将笔者所在医院收治的35例晚期前列腺癌患者采取经尿道前列腺等离子电汽化切除术和睾丸切除术，术后即用比卡鲁胺抗雄激素进行治疗，比较分析治疗前后患者的前列腺症状评分、生活质量评分、PSA、残余尿量、最大尿流率。结果：经随访，术后1年患者的IPSS、QOL评分、PSA、RV、MFR与治疗前比较差异有统计学意义(P<0.01)；术后3年患者的IPSS、QOL评分、PSA、RV、MFR与术后1年时相比，疗效有所下降，但仍显著优于治疗前(P<0.05)。结论：对晚期前列腺癌患者采取经尿道前列腺等离子电汽化切除术联合抗雄激素进行治疗，具有创伤小、出血少、安全有效，可明显提高患者的生活质量，延缓疾病进展，延长寿命，值得临床推广应用。%Objective:To investigate the clinical curative effect of resectoscope plasma electric vaporization resection combined with antiandrogen in the treatment of advanced prostate cancer.Methods:35 cases advanced prostate cancer patients in author’s hospital were given resectoscope plasma electric vaporization resection and orchiectomy,postoperative use bicalutamide antiandrogen treatment,the prostate symptom score,quality of life score,PSA,residual urine volume,maximum urinary flow rate of the patients before and after treatment were compared and analysed.Results:The patients were followed up,compared with before treatment,the IPSS and QOL score,PSA,RV,MFR of patients in 1 year after surgery had statistically significant difference(P<0.01);Follow-up of 3 years operation curative effect in patients had dropped than in 1 year after surgery,but still were significantly better than before treatment(P<0.05). Conclusion:Advanced prostate cancer patients with resectoscope plasma electric vaporization resection combined with antiandrogen treatment,has the virtues of small trauma,less bleeding,safe and effective,and can obviously improve the patient’s quality of life,delay the progression of disease and prolong its service life,it is worth clinical application.
