Objective:To probe into the effect of PDCA combined with 4Y model to reduce the PICC blockage rate in premature infants.Methods:The core team of PICC was established,then to analyze the PICC blockage cause in a total of 40 premature infants received PICC catheter.PDCA circulation combined with 4Y model which included plan in place,responsibility in place,check in place,incentive in place was used in 40 premature infants in observation group,then to compare the difference between both groups.Results:The PICC blockage rate was decreased from 25% to 5%,and the difference was statistically significant (P <0.05).Conclusion:PD-CA combined with 4Y model can significantly reduce PICC blockage rate in premature infants,and will realize the continuous improvement of nursing quality.%[目的]探讨应用 PDCA 循环结合4Y 模式降低早产儿经外周静脉置入中心静脉导管(PICC)堵管的效果。[方法]成立 PICC核心团队，将40例 PICC 置管早产儿中出现堵管的原因进行分析，采用 PDCA 循环结合计划到位、责任到位、检查到位、激励到位的4Y 方法，运用于40例 PICC 观察组早产儿，比较两组之间的差异。[结果]前后两组早产儿 PICC 堵管率从25%降低到5%，差异有统计学意义(P <0.05)。[结论]运用 PDCA结合4 Y管理模式对早产儿 PICC堵管进行研究，堵管率明显降低，实现质量的持续改进。
