Objective To investigate the variation trend of clozapine taken by schizophrenia patients during hospitali-zation of Hebei province in 2002,2006 and 2012. Methods Schizophrenia patients admitted to various levels of hospitals in He-bei province in 2002(543 cases),2006(608 cases)and 2012(2 911 cases)were involved into the study. Self - made ques-tionnaires were used to survey the clozapine taken by the patients in the three years. The using frequency of single clozapine,the using frequency of clozapine combined with other medicines(frequency of combination)and the total using frequency of clozapine in the three years were compared between different hospitals. Results There was a falling trend in using frequency of single cloza-pine,frequency of combination and the total using frequency of clozapine in 2002,2006 and 2012(P < 0. 05). There was a falling trend in using frequency of single clozapine and total using frequency of clozapine among first time inpatients(P < 0. 05). There was a falling trend in using frequency of single clozapine,frequency of combination and the total using frequency of clozap-ine among second time or over inpatients(P < 0. 05). There was a falling trend in using frequency of single clozapine in city and county level hospitals in the three years(P < 0. 05). There was a falling trend in the frequency of combination in city level hospi-tals(P < 0. 05). There was a falling trend in the using frequency of single clozapine among first time inpatients in province,city and county level hospitals(P < 0. 05). The frequency of combination of first time inpatients in province,city and county level hospitals showed no statistically significant difference between the three years(P > 0. 05). There was a falling trend in the using frequency of single clozapine among second time or over patients in city and county level hospitals(P < 0. 05),and there was a falling trend in the frequency of combination in city level hospitals(P < 0. 05). Conclusion There is a falling trend in the use of clozapine among patients with schizophrenia in Hebei province,but different levels of hospitals have different levels of falling, and the first time inpatients do not show obvious falling in the frequency of combination,which should be focused on in clinic.%目的：调查2002年、2006年及2012年河北省精神分裂症患者住院期间氯氮平使用变化趋势。方法以2002年、2006年及2012年在河北省各级精神卫生机构中住院的精神分裂症患者为研究对象，其中2002年543例，2006年608例，2012年2911例。采用自编精神药物使用调查表分别对3年间患者氯氮平使用情况进行调查。比较不同住院次数及不同级别医院3年间单一氯氮平使用频率、合并其他药物氯氮平使用频率（合并氯氮平使用频率）及氯氮平使用总频率。结果2002年、2006年及2012年住院精神分裂症患者单一氯氮平使用频率、合并氯氮平使用频率及氯氮平使用总频率均呈下降趋势（P <0.05）。首次住院患者单一氯氮平使用频率及氯氮平使用总频率均呈下降趋势（P <0.05）。2次及以上住院患者单一氯氮平使用频率、合并氯氮平使用频率及氯氮平使用总频率均呈下降趋势（P <0.05）。3年间市、县级医院单一氯氮平使用频率均呈下降趋势（P <0.05）；市级医院合并氯氮平使用频率呈下降趋势（P <0.05）。首次住院患者省、市、县级医院单一氯氮平使用频率均呈下降趋势（P <0.05）；首次住院省、市、县合并氯氮平使用频率3年间差异无统计学意义（P >0.05）；2次及以上住院患者市、县级医院单一氯氮平使用频率呈下降趋势（P <0.05）；市级医院合并氯氮平使用频率呈下降趋势（P <0.05）。结论氯氮平在河北省住院精神分裂症患者中使用频率呈下降趋势；但不同级别医院首次住院合并氯氮平使用频率下降趋势不明显，应引起临床关注。
