Community health service is a very important part of public health in China. Based on the " separation mode" of health management and basic medical cafe, we have continuously improved the mode and eventually establish the " Dual Mutual Promotion Mode" of community health management. Here we describe and analyze the service process and post of this mode, so as to provide a reference for urban community health workers.%社区卫生服务是我国当前卫生事业的重要内容.本研究在探索健康管理和基本医疗的"分离模式"的基础上,在实践应用中不断丰富完善,建立了"双轨互促"式社区健康管理模式,并对该模式的服务流程及岗位进行详细描述及分析论证,以供社区卫生服务工作者借鉴参考.
