Objective To compare the differences in course of disease, hospitalization time and treatment expense between urban and rural patients with severe mental illnesses. Methods Disease information and medical expenses of inpatients in urban and rural areas in Hebei Province were retrospectively reviewed and analyzed. Results No significant difference was observed in family history, hospitalization frequency, course of disease, treatment effect between the urban and rural groups. The total hospitalization cost, drug cost, examination cost, and average hospitalization time of the urban group were significantly higher and longer than that of the rural group (P <0. 001) . Conclusion Patients with severe mental illnesses living in urban areas have higher hospitalization expense than the rural patients, the government should allocate more financial assistance to rural patients with severe mental illnesses.%目的 比较重性精神疾病管理治疗项目(以下简称“686项目”)城乡示范区重性精神疾病患者的病程、住院时间以及治疗费用等,为国家制定对贫困患者的补助政策提供客观数据.方法 对河北省城乡两个示范区住院患者的疾病资料及住院费用进行分析,总结城乡两个患者群体的患病特点及所需费用.结果 二者在家族史、住院次数、病程、治疗效果上差异均无统计学意义(P＞0.05).城市示范区总费用(6064.5±435.5)元,药费(658.4±90.4)元,检查费用(827.1±71.8)元,平均住院天数(45.8±8.5)d；农村示范区总费用(4 804.8±262.3)元,药费(611.7±56.7)元,检查费用(524.1±51.6)元,平均住院天数(39.4±7.9)d,二者分别比较差异均有统计学意义(P＜0.001).结论 城市示范区患者的住院费用高于农村示范区.农村患者住院时间短,康复时间短,且目前还没有康复站、日间医院等,因此这些患者康复的实施需要在医院的一些康复基地进行,农村患者的住院费用应进一步提高.
