Objective To assess the value of the functional indicators of vascular endothelial cells in prediction of different types of ED and its clinical significance. Methods Toatl of 213 qualified patients, who consulted with sexual dysfunction in andrology out-patient department, were enrolled in this study. Among them, 181 were diagnosed as ED and 32 were normal. According to diagnostic criteria, 181 cases were divided into 3 groups: psychogenic (n=63), vascular (n=57), and the other (n=61). IIEF-5, sVCAM-1, NO and etc. were observed and comparatively analyzed among groups before and after intervention with low dose oral PDE s inhibitor and Yimusake. Results Levels of the functional indicators of vascular endothelial cells in 32 patients without ED and psychogenic ED were significantly different with those in patients with vascular and mixed ED (P < 0.05), and the difference among the individuals in psychogenic ED group was not significant (P> 0.05). After drug treatment, the function of vascular endothelial cells and erectile function in ED patients, especially for psychogenic ED, were gradually improved. Levels of the functional indicators of vascular endothelial cells of the ED group were improved variously afte intervention with low dose oral PDE5 inhibitor and Yimusake. Conclusion The functional indicators of vascular endothelial cells showed a clinical significance in ED diagnosis and therapeutic effect assessment, which merit to be further explored.%目的 探讨血管内皮细胞功能指标对不同类型ED预测,以及疗效判定方面的作用.方法 对男科门诊咨询勃起功能障碍的男性患者进行筛查,在213名符合研究要求的人群中,有181名被诊断为ED,32人为正常.并将181人依据诊断标准分为心因性组(63人)、血管性组(57人)、混合组(61人,其中17名存在内分泌因素)；观察并比较各组人群IIEF-5、sVCAM-1、NO等指标,并对各组ED人群进行小剂量PDE5抑制剂联合维药伊木萨克口服干预.对干预前后相关指标进行比较.结果 32名非ED患者、心因性ED患者的血管内皮细胞功能指标与血管性、混合性ED患者内皮细胞功能检测值比较,差异具统计学意义(P＜0.05),非ED患者与心因性ED患者之间血管内皮细胞功能检测值比较,差异无统计学意义(P＞0.05).但经过药物治疗后,ED患者血管内皮细胞功能指标和勃起功能都存在不同程度恢复,心因性ED患者血管内皮细胞指标改善明显.对ED组人群进行小剂量PDE5抑制剂联合伊木萨克进行干预,干预后血管内皮细胞功能指标得到不同程度恢复.结论 血管内皮细胞功能指标在ED的诊断、疗效判定方面有一定临床意义,值得进一步探讨.
