Objective To observation the effect of U-shape suture combined with tourniquet in treatment of pernicious placenta previa (PPP) bleeding.Methods Totally 96 cases of PPP were selected from obstetrics department of Yan ’ an People’ s Hospital of Shaanxi Province from January 2010 to December 2015 as research subjects .According to surgical method , they were divided into control group ( n=48) and observation group (n=48).Patients in the control group were treated with routine treatment and U-shape suture.On the basis of the treatment for the control group , the observation group underwent tourniquet treatment .The hemostatic effect and the incidence of postoperative complications were compared between two groups .Results The amount of blood transfusion , amount of bleeding during operation and bleeding time of the observation group were obviously better than those of the control group ( t value was 14.846, 18.640 and 7.716, respectively, all P<0.01), and the hysterectomy rate of the observation group was obviously lower than the control group (χ2 =5.275, P<0.05).The amount of vaginal bleeding 24h after operation of the observation group was lower than that of the control group (t=10.432,P<0.01), and the incidence of complications such as puerperal infection , anemia and abdominal incision infection was significantly lower in the observation group (χ2 value was 4.174, 4.909 and 5.194, respectively, all P <0.05).Conclusion The hemostatic effect of U-shape suture combined with tourniquet for PPP is reliable and effective , and it is worthy of clinical application .%目的：观察“U”字缝合联合止血带治疗凶险性前置胎盘（ PPP）出血的效果。方法选取2010年1月至2015年12月于陕西省延安市人民医院产科收治的96例PPP患者作为研究对象，根据手术方法将患者分为对照组（ n＝48）和观察组（ n＝48）。对照组患者行常规处理及“U”字缝合术，观察组在对照组基础上行止血带治疗，比较两组患者术中的止血效果及术后并发症的发生率。结果观察组患者术中输血量、术中出血量、止血时间等指标明显优于对照组（ t值分别为14．846、18．640、7．716，均P＜0．01），且观察组患者子宫切除率明显低于对照组（χ2＝5．275，P＜0．05）。观察组患者术后24h阴道出血量明显低于对照组（t＝10．432，P＜0．01），观察组患者产褥感染、贫血、腹部切口感染等并发症发生率明显低于对照组（χ2值分别为4．174、4．909、5．194，均P＜0．05）。结论“U”字缝合联合止血带治疗PPP止血效果可靠显著，值得临床广泛应用。
