多变量方法在婴幼儿视力筛查结果分析的应用

         

摘要

Objective To study the variation of spherical and cylindrical lens diopter among infants under 3 years old and find vision abnormality early , as well as to explore reasonable selection of statistical analysis methods and explanation of the results according to data characteristics .Methods Infants were examined by Suresight hand-held autorefractor and multivariate statistical analysis methods were selected to analyze the measurement .Results The MANOVA results showed that visual situation was different between two eyes ( mainly caused by the fact that the left eyeball spherical lens was greater than the right ) , and the diopter of three groups were not same .The diopter of infants under 6 months old was highest .The abnormal rate of diopter was between 3.40%and 13.11%, and the cylindrical lens diopter was 28.64%and 70.87%.The abnormal rate of spherical and cylindrical lens diopter was highest in the group under 6 months old.The abnormal rate of spherical lens diopter of infants was higher in left than right (CMHχ2 =5.664, P=0.017).Conclusion The application of MANOVA to analyze the infant vision is feasible .The abnormality of diopter is improved with age increasing , and astigmatism rate is significantly higher than hyperopia or myopia rate .Cases with abnormal spherical and cylindrical lens diopter should be followed up regularly .%目的探索3岁前婴幼儿球径、柱径屈光度变化规律,及早发现婴幼儿视力异常。根据资料特点探索统计分析方法的合理选用及结果的合理解释。方法采用Suresight手持式自动验光仪进行婴幼儿视力筛查,获得球径、柱径屈光度值,并采用多变量统计分析方法对测量结果进行统计分析。结果多元方差分析( MANOVA)显示左眼和右眼的视力屈光度情况不同(主要是由左眼球径大于右眼球径屈光度值所造成的),3个年龄组的视力屈光度情况不全相同且小年龄6个月~组视力屈光度值(球径值、柱径值绝对数)最高。各年龄组球径屈光度异常率介于3.40%~13.11%之间,柱径屈光度异常率介于28.64%~70.87%之间,且球径、柱径视力屈光度异常率均为6个月~组最高。左眼球径异常率高于右眼球径异常率( CMH χ2=5.664,P=0.017)。结论应用多元方差分析等方法分析婴幼儿视力屈光度情况是可行的。婴幼儿视力屈光度异常情况有随年龄增加而减小的趋势,散光率明显高于远视或近视率,球、柱径屈光度异常者应定期随访,动态观察。
