Objective To investigate cause of maternal mortality and changes in its related factors over a decade period in Hanzhong city, Shaanxi province, so as to provide a scientific basis for reducing maternal mortality rate and formulating corresponding interventing measures. Methods The data of maternal mortality in recent ten years in Hanzhong city, data of yearly report in card or booklet forms and case report of investigation on maternal mortality were analyzed retrospectively. Results Over the 10-year period, the maternal mortality rate was 55.16/100 000 in average, which shown a decreasing tendency. In the latter 5 years, the maternal mortality was reduced by 18.53/100 000 (χ2=4.508,P<0.05). The maternal mortality rates in rural areas and mountainous areas were both higher than that in the urban areas. The lower the education level of pregnant women and income of their family and the worse the traffic conditions, the higher the constituent ratioes of maternal mortality. The mortality commonly occurred in prenant women in the 20~30 years age group , multipara, pregnant women with poor health care in gestational period and those who delivered in their home or village hospital. The sequences of the first 3 causes of maternal death were obstetric hemorrhage, complications in internal medicine and amniotic fluid embolism. Conclusion In maternal mortality rate in Hanzhong city in recent 10 years, there is a decreasing tendency. Obstetric hemorrhage ranks the first place in causes of maternal death. In near future, prevention and treatment of obstetric hemorrhage and education of knowledge about maternal and neonatal health should be strengthened. The strategies to reduce maternal mrtality should be explored.%目的 回顾性分析汉中市近10年孕产妇死亡情况,探讨其原因及相关因素变化,为降低孕产妇死亡提供科学依据,制定相应防治措施.方法 对近10年孕产妇死亡资料、年报数据卡册、个案调查报告进行回顾性分析.结果 10年来汉中市孕产妇死亡率平均为55.16/10万,呈下降趋势.后5年较前5年降低了18.53/10万,降低明显(χ2=4.508,P<0.05).农村、山区孕产妇死亡率高于城市,文化程度及经济收入越低、交通越差死亡构成比越高;死亡多发生在20～30岁、经产妇、孕期保健差、在家中和乡村医院分娩的孕妇.死因顺位前3位为:产科出血、内科合并症、羊水栓塞.结论 汉中市近10年孕产妇死亡率呈下降趋势,产科出血占死亡原因首位,针对孕产妇死亡的主要原因今后要加大防治力度和母婴保健宣传教育工作,分析讨论降低孕产妇死亡的对策.
