OBJECTIVE:To investigate the effect of clinical path implemented in Puyang Municipal Oilfield General Hospital ( hereinafter referred to as“our hospital”) on perioperative prophylactic use of antibiotics in patients with thyroid neoplasms.METHODS:By a retrospective study, 150 patients with thyroid neoplasms receiving clinical path management from January to December in 2013 were assigned to the observation group, and another 150 patients with thyroid neoplasms receiving clinical path management during Jan.-Dec.period of 2012 were assigned to the control group.The indications for preoperatively used antibiotics, the choice of antibiotic types, rate of antibiotic use, dosage &administration, timing of the initial administration, course of administration, average length of hospital stay, the total costs and other indicators were compared between the two groups.RESULTS:The observation group showed lower rate of antibiotic use, higher rationality in the choice of antibiotic types and in the time of initial administration and lower average course of administration, lower average length of stay, lower per capita antibiotic cost, per capita drug cost and per capita total costs than in the control group ( P<0.05 ) .CONCLUSIONS:The clinical path can help standardize the perioperative prophylactic use of antibiotics in patients with thyroid neoplasms, shorten the average length of hospital stay and reduce medical costs hence it is worth of popularization.%目的：调查研究濮阳市油田总医院（以下简称“我院”）实施临床路径对甲状腺肿瘤围术期预防使用抗菌药物的影响。方法：采用回顾性调查方法，选择我院2013年1—12月实施临床路径管理的甲状腺肿瘤患者共150例作为观察组，2012年l—12月未实施临床路径的甲状腺肿瘤患者共150例为对照组，比较2组患者围术期抗菌药物适应证、抗菌药选择种类、使用率、用法用量、首剂给药时机、疗程、平均住院时间、总费用等指标。结果：与对照组相比，观察组抗菌药物使用率降低，抗菌药物种类选择合理性明显提高，首剂给药时机合理率增加，平均给药疗程缩短、平均住院时间、人均抗菌药物费用、人均药费、人均总费用降低（ P＜0.05）。结论：临床路径可规范甲状腺肿瘤围手术期抗菌药物的预防使用，缩短患者的平均住院时间，减少医疗费用，值得推广。
