结节性硬化症患儿脑白质的MRI表现和ADC值研究

         

摘要

Objective To explore the measurement value of apparent diffusion coefficient (ADC) and MRI of brain white matter in children with tuberous sclerosis complex (TSC). Methods The MRI diagnostic data and ADC values of 60 children with TSC in our hospital were analyzed retrospectively. The severity of illness and ADC values were analyzed by Spearman correlation analysis, and 30 health children were selected as controls at the same period. Results Except for the temporal lobe and cerebellum of 1~3 years old children with TCS and cerebellum of 12~18 years old children with TSC, the ADC values of temporal lobe, frontal region, occipital region, parietal region and cere-bellum in children at other ages with TSC were significantly higher than those in control group. There were statistically significant differences (P<0.05). The Spearman correlation analysis showed that there was a significant correlation be-tween ADC values of children with TCS and severity of illness (P<0.05). Conclusion Increasing in the ADC values of brain white matter in children with TSC may be related to developmental disorders and cerebral white matter myelin inju-ry. Close observation in the changes of ADC values can be helpful for judging condition and improving the prognosis.%目的:探讨结节性硬化症(Tuberous sclerosis complex,TSC)患儿脑白质的MRI表现及表面弥散系数(Apparent diffusion coefficient,ADC)的测量价值。方法回顾性分析我院收治的60例结节性硬化症患儿的MRI诊断资料及脑白质ADC值,对患儿的ADC值与病情严重程度进行Spearman相关性分析,同时纳入30例同期健康体检儿童作为对照。结果除1~3岁TCS患儿的小脑与颞叶及13~18岁TCS患儿的小脑外,其余年龄TCS患儿的颞叶、额区、枕区、顶区及小脑的ADC值均显著高于对照组,差异均具有统计学意义(P<0.05);Spearman相关性分析显示,TCS患儿的各部位脑白质ADC值与病情严重程度具有显著的相关性(P<0.05)。结论结节性硬化症患儿的脑白质ADC值上升,可能与发育障碍、脑白质髓鞘受损有关,密切观察患儿ADC值的变化,有利于判断病情与改善预后。
