Objective To investigate the therapeutic and aesthetics effects of ultrasound-guided local puncture on congenital preauricular fistula infection ( CPFI) . Methods A total of 110 patients with CPFI from January 2010 to January 2015 were randomly divided into control group ( n=55 ) and observation group ( n=55 ) by means of randomized stratified method. The patients in control group were treated by traditional incision and drainage,however,the patients in observation group were treated by ultrasound-guided local puncture and irrigation combined with microwave therapeutic apparatus. After treatment the abscess cavity healing time,pain disappearance time and incidence rate of adverse reactions were observed and compared between two grousp. Moreover,the patients in both groups were followed up for 1 year,and the recurrence status, scar length and the aesthetic satisfaction rate were observed and compared between two grousp. Results The abscess cavity healing time,pain disappearance time and cure time in observation group were significantly shorter than those in control group (P<0. 05). No severe complications were observed in both groups during treatment,and there were no significant differences between two groups (P>0. 05). The 1-year recurrence rate (1. 8％) in observation group was significantly lower than that (12.7％) in control group (P <0. 05). Moreover there were significant differences in total satisfaction rate (70. 9％ vs 87.3％) and scar length [(1. 59 ± 0. 12)、(0. 48 ± 0. 13)cm] between the two groups (P <0. 05). Conclusion The ultrasound-guided puncture in treatment of congenital ear fistula infection can obtain obvious therapeutic effects, with less trauma,shorter treatment course,lower recurrence rate,which can reduce the scar formation,maintain the facial beauty so as to achieve more satisfactory aesthetic effects,therefor,which is worthy of clinical promotion.%目的 探讨超声引导下局部穿刺治疗先天性耳前瘘管感染(CPFI)的临床疗效及对耳部美观的美学效果分析.方法 选取2010年1月至2015年1月接诊的先天性耳前瘘管感染患者110例,采用随机分层法分为对照组和观察组,每组55例.对照组采用传统切开引流治疗,观察组给予超声引导下穿刺冲洗并配合微波治疗仪治疗;观察2组脓腔痊愈及疼痛消失时间,不良反应发生率,对患者随访1年,观察其复发情况、瘢痕长度及患者对耳部美观满意率.结果 观察组患者脓腔消失、疼痛消失、治愈时间均明显短于对照组(P<0.05).治疗过程中2组均未见严重并发症,组间比较差异无统计学意义(P>0.05).观察组随访1年复发率(1.8％)明显低于对照组(12.7％),组间差异有统计学意义(P<0.05);2组对耳部美观总满意率(70.9％、87.3％)、瘢痕长度[(1.59±0.12)、(0.48±0.13)cm]组间比较差异有统计学意义(P<0.05).结论 超声引导下穿刺治疗先天性耳前瘘管感染疗效显著,具有创伤小、疗程短、复发率低的优势,且能减轻患者瘢痕形成,保持患者面部美观,达到比较满意的美学效果,值得临床推广.
