目的:探讨血清降钙素原(PCT)定量测定在呼吸道与尿路细菌感染诊断中的应用价值.方法:本文收集我院血培养阴性的临床疑似呼吸道感染和尿路感染病例共249例,根据痰培养或中段尿培养结果分组分析同期送检的血清PCT水平,绘制受试者工作特征曲线(ROC曲线)判断血清PCT诊断呼吸道细菌感染和尿路感染诊断性能.血清PCT用罗氏Cobas E601电化学发光免疫检测.结果:痰培养阳性组和尿培养阳性组PCT水平较培养阴性组血清PCT明显升高(P<0.05),差异有统计学意义.以PCT>0.5μg/L为阳性分界值,培养阳性病例中总阳性率为71.96％(59/80),痰、尿培养阳性组与阴性组间比较血清PCT阳性率差异有统计学意义(P<0.05),痰培养阳性和尿培养阳性血清PCT的ROC曲线AUC分别为0.838和0.738.结论:血清PCT水平能作为呼吸道和尿路细菌感染的诊断指标,血清PCT以>0.5μg/L为阳性界值,可以对呼吸道和尿路细菌感染诊断有一定敏感性.%Objective: To evaluate the clinical application significance of serum procalcitonin(PCT)quantitation determination in the diagnosis of respiratory and urinary tract bacterial infections.Methods: 249 cases of clinical suspected respiratory tract or urinary tract infections by Roche Cobas E601 chemiluminescence analyzer(Cobas E601)detected serum PCT level and sputum culture and middle-urine culture.The difference of PCT was analyzed between groups by culture results.Receiver operating characteristic(ROC)curves were drawn to evaluate the diagnostic value of serum PCT.Results: For serum PCT,there was significance for systemic and culture positive groups and negative ones(P<0.05).Being a border of PCT as 0.5μg/L.The PCT positive rate of all cultrue individuals is 71.96％(59/80).The level of PCT was higher in the sputum culture positive groups than that in culture negative groups,as same as middle-urine(P<0.05).The differences of PCT positive rates in groups,by culture results,were significant(P<0.05).The area under the PCT curve of ROC(AUC)for sputum positive groups is 0.838,and middle-urine positive group is 0.738.Conclusion: Inpatients with respiratory and urinary tract bacterial infections,serum PCT level has high expression,and serum PCT has certain application value in judging bacterial infections.
