Objective To analyze the medical charge and influencing factors of inpatients with stroke so as to provide evidence on the reasonable control of medical expenses .Methods Using the data of the first page of medical records of inpatients with stroke in six tertiary hospitals in Shanghai from 2007 to 2010 .Descriptive statisti-cal method was used to analyze the characteristic and rationality of hospitalization costs .The main influential factors were examined using ordinal logistic regression .Results Then median of the medical charge was 10358 .68 Yuan. Among the total expense ,expense for drug was the highest ,accounted for more than 40%.The influencing factor of the medical charge included sex, age, admission condition, length of stay in hospital, complication, results of treatment and the medical insurance payment mode .Conclusion To adjust the structure of medical fee reasona-bly,and reduce the length of stay are helpful to reduce the economic burden of stroke inpatients .%目的：分析脑卒中住院患者费用结构及其主要影响因素，为合理控制医疗费用提供依据。方法利用上海市6个三甲医院2007—2010年脑卒中住院患者病案首页数据，回顾性分析住院费用结构，采用有序Logistic回归方法分析可能与患者住院费用有关的因素。结果20497例脑卒中住院患者平均住院费用为10358．68元。费用构成中药费最高，占40％以上。影响住院费用的因素有性别、年龄、入院情况、住院天数、并发症、治疗结果、费用负担形式。结论应合理调整医疗机构收费结构，缩短住院天数、控制并发症，减轻脑卒中患者的经济负担。
