Objective To analyse the MRI and X-ray findings of lumbar endplate modic changes,investigate the diagnosis value of MRI in classification of modic changes. Method Total 100 patients of lumbar endplate modic changes were retrospective-ly studied,and compared with X-ray. MRI scan with T1WI,T2WI and T2WI-FS( STIR). Retrospectively reviewed the MRI and X-ray findings. Results 381 cases of modic changes were found in 100 patients. Modic changes were divided into typeⅠ,Ⅱ,Ⅲ and mixed. meanwhile,mixed type was further divided into 2 subtypes,namely type Ⅰ ~Ⅱ and type Ⅱ ~Ⅰ. Among them,45 cases(11. 81%)for type Ⅰ,171 cases(44. 88%)for typeⅡ,12 cases(3. 15%)for typeⅢ,115 cases(30. 18%)for mixed typeⅠ~Ⅱ,38 cases(9. 97%)for mixed typeⅡ~Ⅰ. Conclusion MRI can make the correct diagnosis for modic chan-ges,which should be an important role for the classificationg of modic changes and guidance for clinic treatment.%目的：分析腰椎终板modic改变的影像学征象，探讨MRI在腰椎终板modic改变分型中的应用价值。方法：回顾性分析随机抽取的100例患者腰椎终板存在modic改变患者的影像学资料；所有患者均行腰椎矢状面T2WI、T1WI及T2WI反转恢复脂肪抑制（ STIR），并与X线检查进行对照，MRI检查与X线检查时间间隔在一周内，综合分析终板modic改变的信号特点及X线征象，通过双盲法进行modic改变分型并结果统一。结果：100例患者腰椎终板modic改变患者中，共诊断modic改变381处病变。所有病例按modic分型标准分四个类型，即modic Ⅰ型，modic Ⅱ型，modicⅢ型及混合型，其中将混合型又分为混合型Ⅰ~Ⅱ型及混合型Ⅱ~Ⅰ型。所有病例中，modic Ⅰ型共有45处，占11.81％，Ⅱ型171处，占44.88％，Ⅲ型有12处，占3.15％，混合型Ⅰ~Ⅱ型有115处，占30.18％，混合型Ⅱ~Ⅰ型，有38处，占9.97％。结论：MRI可准确地诊断腰椎终板modic改变，在腰椎终板modic改变的分型中具有较高的价值，有助于指导临床对该病的诊治。
