目的:探讨季德胜蛇药片外敷剂量对蛇咬伤后局部肿胀的影响及护理体会.方法:选取90例毒蛇咬伤的病人,随机分为低、中和高剂量组,3组病人均给予合适的抗蛇毒血清治疗,并进行破伤风预防、抗感染、抗纤溶、改善微循环等常规治疗.在此基础上,3组病人分别给予低、中和高剂量季德胜蛇药片外敷,同时均给予良好的临床护理.比较3组病人的临床疗效、主要症状消失时间及住院时间等.结果:经治疗后,高剂量组临床疗效有效率为96.7％,均高于中、低剂量组(P<0.05和P<0.01),中剂量组次之为80.0％,亦高于低剂量组的63.3％(P<0.05);病人局部红肿消失时间、疼痛消失时间、伤口愈合时间和住院时间均为高剂量组少于中、低剂量组(P<0.05～P<0.01),中剂量组亦少于低剂量组(P<0.05～P<0.01).结论:在常规治疗及护理的基础上,采用不同剂量季德胜蛇药片外敷均可有效缓解毒蛇咬伤后局部肿胀,且高剂量蛇药片临床效果更好.%Objective:To explore the effects of Jidesheng Sheyao tablet topical concentration on the swelling caused by snakebite,and nursing experience. Methods:Ninety patients with snakebite were randomly divided into the low dose group,medium dose group and high dose group. Three groups were treated with appropriate antivenom,preventing tetanus,anti-infection,anti-fibrinolysis and improving microcirculation. Three groups were additionally treated with low dose,medium dose and high dose of Jidesheng Sheyao tablets,and good nursing,respectively. The clinical efficacy, main symptoms disappearing time and hospitalization time between three groups were compared. Results:After treatment,the total effective rate in low,medium and high dose groups were 63. 3％,80. 0％ and 96. 7％, respectively(P<0. 05). The local swelling disappearing time,pain disappearing time,wound healing time and hospitalization time in high,medium and low dose groups gradually prolonged in turn(P<0. 05 to P<0. 01). Conclusions:On the basis of routine treatment and nursing,the different concentrations of Jidesheng Sheyao tablets can effectively alleviate the external swelling of snakebite,and the better clinical efficacy,the higher dose of tablet we will porate patients.
