Objective To explore the factors influencing depression patients to receive psychiatric ambulatory treatment. Methods A total of 158 depression patients were assigned to psychiatric (n=58) and non-psychiatric group (n=100) according to whether their first-visits were at psychiatric out-patient department,factors influencing depression patients to receive psychiatric ambulatory treatment analyzed,health guidance was given to them,and all patients were followed up for 1 year. Results 63.3 percent of the patients selected non-psychiatric out-patient department when they first visited doctor;the probability of ≤60 days from onset to first-visit was significantly lower (P<0.01) and first-visit medication ≥ 180 days,concordance rate of diagnoses,appropriate rate of medication and total effective rate higher (P<0.01) in psychiatric than in non-psychiatric group. The probability of being ignorant of suffering from depression and of where to visit among causes of first-visits weren't at psychiatric out-patient department was significantly lower in psychiatric than in non-psychiatric group (P<0.01);understanding degrees of depression-related knowledge and medication compliance after health guidance heightened more significantly (P<0.01). Conclusion Lack of mental disorder knowledge and un-valuing mental health are key factors influencing prompt visit at psychiatric out-patient department and the patients to acquire prompt and effective treatment,active propaganda and education of mental health knowledge to boost the realization about mental disorder knowledge are beneficial to early diagnosis and treatment.%目的 探讨抑郁症患者接受精神科门诊治疗的相关影响因素.方法 将158例抑郁症患者初诊是否在精神科接受诊治分为两组,其中精神科组(A组)58例,非精神专科组(B组)100例.采取自制问卷调查方式对影响接受专科治疗的相关因素进行分析,并对患者进行健康指导,所有患者随防1 a.结果 63.3%的抑郁症患者在初诊时选择了非精神科;A组从患病到初诊时间≤60 d的概率显著低于B组(P<0.01),首诊用药≥180 d、诊断一致率、用药恰当率、总有效率均显著高于B组(P<0.01).A组未立即到精神专科就诊原因中不知道患有抑郁症、不知道何处就医的比例显著低于B组(P<0.01);患者接受健康指导后对抑郁症相关知识的了解程度及服药依从性较健康指导前显著提高(P<0.01).结论 人们对精神疾病知识的缺乏、对精神卫生的不重视是影响抑郁症患者到精神科门诊及时就诊,获得及时、有效治疗的关键因素,积极开展精神卫生知识宣传与教育,提高人们对精神疾病的认识,有利于患者的早诊断、早治疗.
