循环microRNA-1268在肺癌血浆中检测分析及临床意义

         

摘要

目的:探讨循环microRNA-1268(miRNA-1268)在肺癌血浆中的表达情况,并分析其与临床病理特征的相关性及其在肺癌诊断及预后判断中的意义.方法:采用实时荧光定量PCR方法检测miRNA-1268在39例肺癌患者和作为对照组的39例正常人血浆中的相对表达量,并评估其临床意义.结果:肺癌血浆中miRNA-1268的表达水平明显高于正常人,差异有统计学意义;血浆循环miRNA-1268相对表达量ROC曲线下面积(area under the ROC curve,AUC)为0.680,95%CI为0.562-0.799,取临界值0.054时,血浆miRNA-1268表达诊断肺癌的灵敏度为74.4%,特异度为59.0%;miRNA-1268的表达水平与肿瘤TNM分期相关,而与患者性别、年龄、吸烟、肿瘤大小、淋巴结转移、组织分型、CEA水平无相关性;Kaplan-Meier分析结果显示肺癌血浆中miR-1268高表达者生存时间明显低于低表达者,差异有统计学意义.结论:循环miRNA-1268在肺癌血浆中表达上调,对肺癌有一定的临床诊断及预后判断意义,可作为诊断肺癌和判断预后的一个新的标志物.%Objective To discuss the expression of circulating miRNA-1268 in lung cancer plasma and to analyze its di-agnostic and prognostic evaluation value in lung cancer. Methods Real-time fluorescent quantitative PCR was used to detectthe relative expression level of circulating miRNA-1268 in 39 lung cancer patients (cancer group) and 39 healthy people (con-trol group). Results The expression of miRNA-1268 in the cancer group was up-regulated compared with the control group. The area under the ROC curve was 0.680, and the 95%CI was 0.562~0.799, its sensitivity in the diagnosis of lung cancer was 74.4%and its specificity was 59.0%when the relative expression level of circulating miRNA-1268 was 0.054. The expression of miRNA-1268 was related to tumor stage, and no obvious correlation was found between the expression of miRNA-1268 and some pathological characteristics such as gender, age, smoke, CEA, tumor size, lymph node metastasis and tissue typing. Kaplan and Meier analysis results show that there was significant difference between the expression of plasma miR-1268 and overall survival of lung cancer, patients with high expression of miR-1268 showed shorter survival than those with low expression of miR-1268. Conclusion The up-regulated expression of circulating miRNA-1268 has certain value in the diagnosis and prog-nostic evaluation of lung cancer, and it may be a useful diagnostic and prognostic biomarker for lung cancer.
