肥胖儿童血管损害与速度向量成像技术应用初步研究

         

摘要

目的:探讨速度向量成像技术应用于肥胖儿童血管损害中的作用。方法选取10~14岁150例体检者按照体重指数分组为甲乙丙三组,各50例,其中甲组为正常组、乙组为超重组、丙组为肥胖组。采用速度向量成像技术检测三组颈总动脉收缩期最大径向应变(Smax)和舒张期最小径向应变率(SRmin)及收缩期最大径向应变率(SRmax),然后计算SRmax与SRmin间之差,同时分析SRmax、SRmin及△SR与体重指数( BMI)间关系。结果甲、乙、丙三组体重指数分别为(16.4±1.8)kg/m2、(21.3±1.7)kg/m2、(26.3±3.0)kg/m2,三组间比较( P <0.05)。甲组与丙、乙组颈总动脉各壁段Smax相比,其中甲、乙组比较( P <0.05);甲、丙组比较( P <0.05;丙、乙组比较( P <0.05)。乙丙组颈总动脉各壁段SRmax明显低于甲组( P <0.05);其中甲、乙组比较( P <0.05);甲、丙组比较( P <0.05);乙、丙组比较( P <0.05)。甲组颈总动脉各壁段△SR明显高于乙、丙组( P <0.05);其中甲、乙组比较( P <0.05);甲、丙组比较( P <0.05);乙、丙组比较( P <0.05)。其中△SR、Smax及SRmax与体重指数呈负相关性( r =-0.739,-0.623,0.679, P <0.05)。结论速度向量成像技术应用于肥胖儿童血管中可有效评价因肥胖或超重所致血管弹性下降,从而为监测心血管疾病提供重要参考。%Objective To explore the role of velocity vector imaging technology in assessment of vascular damage in obese children .Methods 150 children aged from 10 to 14 years old were divided into three groups according to the body mass index:group A, group B, group B, group C, respectively.The maximal radial strain (Smax), the minimum radial strain rate (SRmin) and the maximal radial strain rate ( SRmax) of the systolic phase were measured by the velocity vector imaging technique .Then, SRmax and SRmin were calculated .Meanwhile , the relationship among SRmax , SRmin and△SR with body mass index ( BMI) was analyzed.Results The body mass index in three groups ( A, B, C) was (16.4 ±1.8) kg/m2, (21.3 ±1.7) kg/m2, (26.3 ±3.0) kg/m2 , respectively, with statistically significant difference in body mass index among the three groups ( P <0.05 ) .For the comparison of Smax in each segment of carotid common artery , compared the Smax in group A vs B , P value was less than 0.05, in group A vs C, P value was less than 0.05, and in group C vs B, P value was less than 0.05.For the compari-son of SRmax of carotid common artery in group B and C , SRmax was significantly less than that in group A .SRmax in group A vs B, P value was less than 0.05, in group A vs C, P value was less than 0.05, and in group C vs B, P value was less than 0.05 .For the comparison of△SR of carotid common artery ,△SR in group was significantly less than that in group B or C .△SR in group A vs B, P value was less than 0.05, in group A vs C, P value was less than 0.05, and in group C vs B, P value was less than 0.05.△SR, Smax, and Rmax were negatively correlated to body mass index ( r =-0.739, -0.623, 0.679, P <0.05 ) .Conclusion Velocity vector imaging can be used to evaluate the vascular elasticity of obese children , thereby providing an important reference for the monitoring of cardiovascular disease .
