用重建心阻抗图测量心脏效率和内功的初步研究

         

摘要

目的 探讨无损伤测量心脏效率和内功的方法.方法 基于生理学的相关资料和大量测量数据的统计分析,提出测量心脏效率和内功的计算方法,测量180例健康成人志愿者和181例心血管患者(高血压病患者51例,冠心病患者58例,心功能不全患者72例)的重建心阻抗图,用其相关波形参数计算心脏效率、内功、心输出量等,利用统计软件 SPSS12.0完成统计分析.结果 180例健康志愿者的心脏效率和内功分别为(22.7±2.0)%和(242.6±62.2)J·min-1;心脏效率随年龄的增大而减小,内功则随年龄增大而增大,且青年组与老年组之间有显著性差异[(218.8±45.1)J·min-1比(266.9±66.4)J·min-1,P<0.01];男性组和女性组心脏效率比较差异无统计学意义(P>0.05),男性组的内功显著大于女性组(P<0.01).高血压病、冠心病、心功能不全患者的心脏效率分别为(16.9±2.7)%、(19.1±2.4)%、(17.2±3.2)%,均显著小于老年组(P<0.001).结论 本文测量心脏效率和内功的方法是可行的.%Objective To investigate the noninvasive method for measuring the cardiac efficien-cy and internal work.Methods The computed method for measuring the cardiac efficiency and in-ternal work was suggested on the basis of physiological data and statistical data analysis.The re-constructed impedance cardiograms were measured in l80 normal adults and 181 patients with car-diovascular disease(51 patients with hypertension,58 patients with coronary heart disease and 72 patients with cardiac insufficiency).Cardiac efficiency,internal work and cardiac output were cal-culated using the relative waveform parameters.The statistical analysis was performed using the SPSS12.0 software.Results The cardiac efficiency and internal work were(22.7 ± 2.0)% and (242.6±62.2)J·min-1in the 180 normal adults,respectively.The cardiac efficiency decreased and the internal work increased with the age.There was a notable difference in internal work be-tween youth group[(218.8±45.1)J·min-1]and elderly group[(266.9±66.4)J·min-1](P<0.01).No significant difference in cardiac efficiency was found between male group and female group(P>0.05).However,the internal work in male group was obviously greater than that in fe-male group(P<0.01).The cardiac efficiencies in patients with hypertension coronary,patients with heart disease and patients with cardiac insufficiency were,respectively(1 6.9 ± 2.7)%,(1 9.1 ± 2.4)% and(17.2±3.2)%,markedly lower than those in elderly group(P<0.001).Conclusion The method suggested in this paper is feasible for measuring cardiac efficiency and internal work.
