Objective To make a systematic evaluation of the efficacy and safety of Eszopiclon and Alprazolam in the treatment of insomnia.Methods By searching the PubMed,Cochrance Library,CNKI, WANFANG,and VIP database,we studied the literature published between 2005 to 2016 on the efficacy and safety of Eszopiclon and Alprazolam in the treatment of insomnia.We collected the randomized controlled trials (RCTs), evaluated the quality of methods and then analyzed the data using RevMan5.3 software.Results A total of 18 RCTs were included,involving 2088 patients.According to Meta-analysis,Eszopiclon group had a significantly higher total efficacy rate [RR=1.07,95% CI (1.02,1.11),P=0.003],lower severity of adverse reactions [MD=-0.43,95% CI(-0.75,-0.12),P=0.008]and incidence of adverse reactions [RR=0.46,95% CI(0.32, 0.66),P<0.0001]than Alprazolam group.However,the two groups did not significantly differ in sleep quality scores after 4-week teatment [MD=-0.05,95% CI(-0.22,0.12),P=0.54].Conclusion Eszopiclon is more effective on insomnia than Alprazolam,with better safety,and it deserves wide clinical use.%目的 系统评价右佐匹克隆对比阿普唑仑治疗失眠症的疗效与安全性.方法 计算机检索数据库(PubMed、Cochrance Library、CNKI、WANGFANG、VIP等),查阅自2005-2016年公开发表的有关右佐匹克隆对比阿普唑仑治疗失眠症的相关文献.并按照纳入和排除标准对文献进行筛选,并对所筛选的文献进行方法质量评价,使用RevMan5.3软件对纳入的文献进行数据分析.结果 纳入18篇随机对照试验文献,共计2088例患者.Meta分析结果显示,在治疗失眠中,右佐匹克隆与阿普唑仑相比,能提高总有效率[RR=1.07,95%CI(1.02,1.11),P=0.003],降低不良反应的严重程度[MD=-0.43,95%CI(-0.75,-0.12),P=0.008]和不良反应发生率[RR=0.46,95%CI(0.32,0.66);P<0.0001],且结果具有统计学意义,而对于治疗4周的匹兹堡睡眠质量评分没有统计学差异[MD=-0.05,95%CI(-0.22,0.12),P=0.54].结论 右佐匹克隆治疗失眠症的疗效优于阿普唑仑,且安全性更好,值得临床推广使用.
