目的:对复合皮移植和皮肤软组织扩张术在烧伤患者的整形治疗中的应用效果和安全性进行对比分析.方法:选择烧伤患者100例,随机分成观察组和对照组,每组患者均为50例.对照组进行复合皮移植治疗,观察组使用皮肤软组织扩张术进行治疗.对比两组患者的疗效、皮肤创面愈合时间、皮肤血运恢复时间及术后并发症.结果:观察组患者的总有效率为94%(47／50) ,对照组患者的总有效率为78%(39／50) ,两组患者的总有效率具有统计学差异( P＜0.01).观察组患者的平均皮肤创面愈合时间为(11.32 ± 3.1)d ,对照组则为(15.87 ± 4.5)d.观察组的皮肤血运恢复时间为(19.02 ± 4.6)d ,对照组则为(24.21 ± 5.1)d.这两者的数据差异均具有统计学意义( P＜0.01).另外,观察组的术后并发症的发病率(10%)要低于对照组的并发症发生率(24%) ,差异有统计学意义(P＜0.01).结论:和复合皮移植相比,皮肤软组织扩张术在烧伤患者的整形治疗中具有更好的效果,更短的创面愈合时间,更短的血运恢复时间以及更低的并发症发生率.%Objective:To compare and analyse the therapeutical effect and safety ofcomposite skin grafting and skin soft tissue expansion technique in the treatment of burn and plastic patients.Methods:From December,2013to December,2017,100burn patients were selected in our hospital andrandomly divided into control group and observa-tion group,50 cases in each group.The patients in the control group were given composite skin grafting treatment, and patients in the observation group experienced skin soft tissue expansion surgery.The clinical efficacy of these two types of treatemnt were evaluated,the wound healing time and the wound skin blood transportation recovery time were compared between these two groups.Besides,the total postoperative complication rate was compared in these two groupsafter surgery.Results:After treatment, the clinical efficacy rate of patients inthe organization group was94%(47/50),which was significantly higher than that in the control group(39/50,78%).The the wound heal-ing time of the patients in the control group was (15.87 ± 4.5)d,while that in the patients givenskin soft tissue ex-pansion surgery was(11.32 ± 3.1)d,much less,obviously.As to the other idex-the wound skin blood transporta-tion recovery time,there’s statistic difference in these two operation type,with the time was (19.02 ± 4.6)d in the observation group and (24.21 ± 5.1)d in the control group(P＜0.01).Besides,the total postoperative complication rate of the observation group(10%)was significantly lower than that in the control group(24%)(P＜0.01).Conclu-sion:Skin soft tissue expansion techniquein can achieve shorter wound healing timeand wound skin blood transporta-tion recovery time and better clinical efficacy rate with less postoperative complication rate,may be a good choice of therapeutic scheme.
