目的：探讨MRI对血管压迫性三叉神经痛责任血管的显示及诊断价值。方法：回顾性分析38例血管相关性三叉神经痛患者的临床及MRI资料，所有患者均行常规头颅MR平扫、桥小脑角区高分辨3DTOF和3DFIESTA序列扫描。分析MRI显示病变责任血管，并追踪手术结果进行对照。结果：发现责任血管，右侧22例，左侧16例，分别为小脑上动脉25例，小脑下前动脉9例，小脑下后动脉3例，椎动脉1例。结论：MRI3DTOF和3D-FIESTA序列对血管压迫性三叉神经痛的责任血管的显示有重要意义，有助于临床术前准备和评估。%Objective:To investigate the capacity and diagnostic value of MRI in patients with vascular compressive trigeminal neuralgia .Methods:The clinical materials and MR images of 38 patients with trigeminal neu-ralgia caused by the offending vessels were analyzed retrospectively .All patients were scanned with conventional se-quences including SE T1WI ,T2WI ,FLAIR of head ,And High resolution 3 D TOF MRA scanning and 3 D FIESTA sequence .Results :The offending vessels ,22 cases on the right side ,16 cases on the left side of the ,The responsi-ble vessels are as follows :superior cerebellar arteries in 25 patients ,9 cases in anterior inferior cerebellar artery and 3 cases in posterior inferior cerebellar artery ,1 case in vertebral artery Conclusion :MRI could display the the offen-ding vessels of trigeminal neuralgia sensitively and accurately ,definite the relationship of the neurovascular and pro-vide guide for operation .
