外周血中25-(OH)D3 IgE浓度水平在喘息型支气管炎患儿发病机制中的相关性分析

         

摘要

目的 通过对喘息型支气管炎患儿、正常婴幼儿、支气管肺炎患儿外周血中25-(OH)D3、IgE浓度的测定,分析其浓度水平的变化在喘息型支气管炎患儿的发病机制中的相关性,以期为防治此类型支气管炎提供理论参考.方法 随机选择我院2015年2月至2016年2月的诊断标准为喘息型支气管炎患儿共80例,定为喘支组(80例);作为对照选择同期入院的诊断标准符合支气管肺炎的患儿80例,定为肺炎组(80例);选择在我院健康体检的婴幼儿80名为健康对照组.使用真空采血管抽取健康体检的婴幼儿及入院患儿第1天的外周静脉血2 mL,所采血液应为空腹抽取,抽血前禁止用药.用3 000 r/min的离心机离心静置1 h的血液15 min,然后取血清保存在EP管中,置于-80 ℃的低温环境下.就可以应用ELISA法来检测EP管待测的血清中的25-(OH)D3和 IgE的浓度.结果 经比较,在外周血中,对于25-(OH)D3的浓度水平,喘支组比肺炎组有所降低,肺炎组又比对照组明显偏低,喘支组与对照组相比也明显降低,差异有统计学意义(P<0.05);在外周血中,对于IgE的水平,肺炎组比喘支组有所降低,喘支组则比对照组明显偏高,差异有统计学意义(P<0.05),肺炎组比对照组差异无统计学意义(P>0.05).结论 ①喘支组和肺炎组相比较,其血清中的 IgE 和25-(OH)D3的浓度水平的相关性表现为负相关,但是在对照组中,则无相关性.常见于婴幼儿的喘息性疾病的发生与外周血中维生素D的水平降低有关.②缺乏维生素D会改变体内IgE水平,进而引发喘息性疾病.%Objective To determine the concentration of 25-(OH) D3 and IgE in peripheral blood of children with asthmatic bronchitis, normal infants and children with bronchial pneumonia.To analyze the changes of plasma concentration of 25-(OH) D3 and IgE in children with asthmatic bronchitis in the pathogenesis of bronchitis in order to provide a theoretical reference for the prevention and treatment of this type of bronchitis.Methods A total of 80 children with asthmatic bronchitis were randomly selected from our hospital from February 2015 to February 2016.Eighty patients were selected as asthmatic bronchitis group.The diagnostic criteria of hospital admission was consistent with bronchopneumonia (80 cases), and those 80 cases were selected as the control group.The vacuum blood collection tube was used to take 2 mL peripheral blood of healthy physical examination of infants and children admitted to the first day, the blood should be taken under fasting, and without any medication.Sample were centrifuged at 3 000 r/min for 15 minutes, then were stored in an EP tube and place in a low temperature of minus 80 degrees.The concentration of 25-(OH)D3 and IgE in the serum of the EP tube to be measured can be measured by ELISA.Results Compared with the control group, the concentration of 25-(OH)D3 in the peripheral blood was lower than that in the pneumonia group and the pneumonia group was lower than that in the control group(P<0.05).In the peripheral blood, the level of IgE in pneumonia group was lower than that in asthmatic group, while that in asthma group was significantly lower than that in control group (P<0.05), and the difference was significant (P<0.05).Compared with the control group the increased in the pneumonia group is not statistically significant (P>0.05).Conclusion There was a negative correlation between serum IgE and 25-(OH) D3 concentration level, in asthma group and pneumonia group, but no correalation was found compared with the control group.Frequent wheezing in infants and young children is associated with a decrease in the level of vitamin D in the peripheral blood.The lack of vitamin D will change the body's IgE levels, and then trigger a wheezing disease.
