ObjectiveTo investigate laparoscopic transabdominal preperitoneal procedure(TAPP)value in the treatment of bilateral inguinal hernia.MethodsFrom July 2013 to December 2014 in our hospital,we performed TAPP treatment of bilateral inguinal hernia 21 cases. We established pneumoperitoneum after anesthesia,and satisfied the hernia contents back,and separated preperitoneal space,and free hernia,and free the spermatic cord or uterine round ligament,pubic separation gap(Retzius space)and the groin after a gap(Bogrus gap). At last the patch is complete coverage of bilateral pubic muscle hole,and weclosed the peritoneum cracks.Results21 cases were successfully completed by laparoscopic transabdominal preperitoneal procedure,including eight cases of intraoperative findings concealed hernia. The operation time 73~115min,the average 95min. The blood loss was 10 ~ 80 ml,average 32ml. Two cases developed hernial sac,and mass disappeared after subcutaneous aspiration. One case suffer from urinary retention,2d after indwelling catheter removal. Serious complications futile sheet rejection,no vital organs,vascular damage,no intestinal obstruction,abdominal hernia occurred. 19 cases were followed up 8 to 26 months,an average of 18 months,no recurrence.ConclusionsTAPP has its irreplaceable value bilateral inguinal hernia surgery in other waysand is safe,effective and feasible for bilateral inguinal hernia or suspected patients with bilateral inguinal hernia should be preferred.%目的：探讨经腹腹膜前腹腔镜疝修补术（TAPP）在双侧腹股沟疝治疗中的应用价值。方法2013年7月至2014年12月行TAPP治疗双侧腹股沟疝21例，全身麻醉后建立气腹，回纳疝内容物，分离腹膜前间隙，游离疝囊，腹壁化精索或子宫圆韧带，分离耻骨后间隙（Retzius间隙）和腹股沟后间隙（Bogrus间隙），将补片完整覆盖双侧耻骨肌孔，关闭腹膜裂口。结果21例均成功完成经腹膜前腹腔镜下双侧腹股沟疝修补，其中8例为术中发现对侧隐匿性疝。手术时间73~115min，平均95min。术中出血量10~80 ml，平均32ml。术后疝囊内血清肿2例，皮下穿刺抽吸后包块消失；尿潴留1例，留置尿管2d后拔除。无补片排异反应，无重要脏器、血管损伤，无粘连性肠梗阻、腹内疝等严重并发症发生。19例随访8~26个月，平均18个月，无复发。结论 TAPP在双侧腹股沟疝的诊断和治疗中具有其他手术方式不可替代的特殊价值，是安全、有效、可行的方法，对于双侧腹股沟疝或怀疑双侧腹股沟疝的患者应首选。
