Objective To analyze the risk factors of delirium from the perspective of blood biochemical markers. Methods 1205 patients were hospitalized in the department of critical care medicine from December 2015 to April 2017.These patients were divided into observation group (delirium,n=420 cases) and control group (without delirium,n=785 cases) and the differences in blood biochemical markers between the two groups were compared. Results The incidence of delirium was 34.85％(420/1205).Univariate analysis revealed that the two groups of C reactive protein,procalcitonin PCT,cortisol,urea nitrogen,sodium ion,magnesium ion,hemoglobin,aspartate aminotransferase,alanine aminotransferase,lactate 10 blood indexes,P value was less than 0.05,there was significant difference. Multivariate analysis had 8 indicators into the logistic regression equation. The results were as follows:Procalcitonin(OR:1.958,95％ CI:1.255～3.056),cortisol(OR:1.029,95％ CI:1.021～1.037),blood urea nitrogen(OR:2.352,95％ CI:1.816～3.046),sodiumion(OR:1.097,95％ CI:1.041～1.156),hemoglobin (OR:0.981,95％ CI:0.963～0.999),glutamic-oxalacetic transaminase(OR:1.142,95％ CI:1.099～1.187),glutamic-pyruvic transaminase (OR:1.087,95％ CI:1.054～1.120),lactic acid(OR:1.375,95％ CI:1.080～1.751). Conclusion It is significant to monitor the changes of blood biochemical markers to predict the occurrence of delirium.%目的 通过谵妄患者血液指标分析,探讨谵妄发生的高危生化因素.方法 选择2015年12月至2017年4月重症医学科住院患者1205例,按是否发生谵妄分为观察组(发生谵妄,420例)和对照组(未发生谵妄,785例),比较两组患者血液指标方面差异.结果 谵妄发生率为34.85％(420/1205),单因素分析发现,两组C反应蛋白,降钙素原PCT,皮质醇,尿素氮,钠离子,镁离子,血红蛋白,天冬氨酸转氨酶,丙氨酸转氨酶,乳酸等,差异有统计学意义(P<0.05).多因素分析8个指标进入logistic回归方程,降钙素原PCT(OR:1.958,95％CI:1.255～3.056),皮质醇(OR:1.029,95％CI:1.021～1.037),尿素氮(OR:2.352,95％CI:1.816～3.046),钠离子(OR:1.097,95％CI:1.041～1.156),血红蛋白(OR:0.981,95％CI:0.963～0.999),天冬氨酸转氨酶(OR:1.142,95％CI:1.099～1.187),丙氨酸转氨酶(OR:1.087,95％CI:1.054～1.120),乳酸(OR:1.375,95％CI:1.080～1.751)是谵妄发生的高危生化因素.结论 监测血液指标变化对预测谵妄发生有重要参考意义.
