目的 调查平谷地区布氏杆菌病流行病学特点,为诊断和治疗提供可靠依据.方法 回顾性分析2007~2016年平谷地区布氏杆菌病患者的流行病学资料,对临床诊断病例血培养特点和布氏杆菌凝集试验结果进行分析.结果 2007~2016年,平谷地区布氏杆菌病发病人数依次为为1、0、0、1、2、5、11、29、60、49.2014~2016年平谷区医院共发现108例布氏杆菌病患者,其中102例为首诊.90例血培养和布氏杆菌凝集试验同步采血检验的患者,凝集试验阳性90例,阳性率100%.单套血培养需氧瓶阳性63例,阳性率70%,厌氧瓶全部阴性.63例血培养阳性报警时间36.2~128.6h,平均报警时间68.59h.结论 布氏杆菌病发病率与患者职业有明显相关性.布氏杆菌病症状复杂,极易误诊.诊断布氏杆菌病,凝集试验阳性率虽高于血培养,但血培养的特异性更高.由于感染后抗体可持续阳性>2年,对于复诊患者,在进行布氏杆菌凝集试验的同时,应进行血培养检查.%Objective To investigate the epidemiological characteristics of brucellosis in Pinggu,and to provide reliable evidence for diagnosis and treatment.Methods The epidemiological data of brucellosis patients in Pinggu area in 2007~2016 were analyzed retrospectively,and the characteristics of blood culture and the results of brucellosis agglutination test were analyzed.ResultsThe incidence of brucellosis in Pinggu were 1,0, 0,1,2,5,11,29,60 and 49,respectively. 108 cases of brucellosis patients were found in Pinggu district hospital from 2014 to 2016,including 102 cases of first diagnosis. Of 90 blood cultures and Brucella agglutination tests,90 patients were positive for agglutination test,and the positive rate was 100%. The positive aerobic bottle was 63 cases in single blood culture,the positive rate was 70%,and the anaerobic bottle was all negative. 63 cases were positive for blood culture,the alarm time was 36.2~128.6h,and the average alarm time was 68.59h.ConclusionsThe incidence of brucellosis is significantly related to the occupation of the patients. Brucellosis has complicated symptoms and is easily misdiagnosed. The positive rate of brucellosis is higher than that of blood culture,but the specificity of blood culture is higher. Since the antibody after infection is positive for more than 2 years,blood cultures should be performed at the same time as the brucellosis agglutination test is performed.
