增强CT MRI及超声造影对肾脏占位性病变的诊断价值比较

         

摘要

目的:比较增强CT、增强MRI以及超声造影对肾脏占位性病变的诊断价值。方法2011年5月至2014年5月期间经手术病理诊断为肾脏占位性病变80例患者(86个病灶)。所有患者均行CT、MRI以及超声检查,其中行超声造影病灶80个,行增强CT病灶41个,增强MRI病灶35个,同时行增强CT、增强MRI病灶患者8个。研究分析增强CT、增强MRI以及超声造影在肾脏占位性病变中的诊断价值。结果80例患者(86个病灶),恶性病灶52个、良性病灶34个。增强CT、增强MRI以及超声造影诊断的阳性准确率分别为92.86%、87.5%、91.38%,三种方法比较阳性准确率无明显差异(P均>0.05)。增强CT诊断灵敏度和特异性分别为92.31%、73.33%;增强MRI诊断灵敏度和特异性分别为95.24%、71.43%;超声造影诊断灵敏度和特异性分别为94.23%、73.53%;三种方法灵敏度、特异性比较无明显差异(P均>0.05)。结论在肾脏占位性病变的临床诊断中,增强CT、增强MRI以及超声造影诊断价值相当,无显著差异,对于患者不同情况使用适当的诊断方法更为适合。%Objective To compare the diagnosis value of strengthening CT,MRI and ultrasound contrast in the diagnosis of renal space occupying lesions. Methods 80 cases of patients who were diagnosed as renal occupying lesions from May 2011 to May 2014 were selected,there were 86 lesions in total. All patients were given CT,MRI and ultrasound examination,and 80 lesions were performed by ultrasound,there were 41 CT enhancing lesion,35 MRI enhancing lesion,at the same time,8 patients with CT enhancing lesion and MRI enhancing lesion were performed. The diagnostic value of contrast-enhanced CT,enhanced MRI and contrast-enhanced ultrasound in the diagnosis of renal space occupying lesions were analyzed. Results There were 86 patients with 80 lesions,52 malignant lesions and 34 benign lesions. The positive rates of CT,enhanced MRI and contrast-enhanced ultrasound were respectively 92.86%,87.5% and 91.38%,there were no differences between the three Methods,the positive rates were P>0.05. The diagnostic sensitivity and specificity of enhancing CT were respectively 92.31% and 73.33%;The diagnostic sensitivity and specificity were 94.23% and 73.53%,respectively. And the sensitivity and specificity of the three methods were not different,P>0.05. Conclusion In clinical diagnosis of the renal space occupying lesions,diagnostic values of enhanced CT and enhanced MRI and diagnostic value of contrast-enhanced ultrasound diagnosis are similar,there are no differences,appropriate diagnostic Methods for patients with different conditions are more appropriate.
