目的：探讨老年人颈动脉斑块的相关因素，分析肾小球滤过率（eGFR）、纤维蛋白原（FIb）及脉压（PP）、空腹血糖（FPG）、超敏C-反应蛋白（Hs-CRP）与颈动脉斑块的关系及其对心血管疾病的影响。方法将2012年9月至2013年12月期间因动脉硬化性相关疾病的老年患者124例，进行颈动脉彩色多普勒超声检查，根据有无颈动脉斑块分组进行分析。其中有颈动脉斑块为观察组74例，无颈动脉斑块为对照组50例，对两组进行FPG、总胆固醇（TC）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）、FIb、PP等指标测定并估算eGFR。结果观察组PP、FPG、Hs-CRP、FIb较无斑块组高，差异均有统计学意义（P<0.05）。两组eGFR差异无统计学意义（P=0.67）。Logistic回归分析提示脉压与颈动脉斑块呈线性相关（P=0.05）。结论 PP、FPG、Hs-CRP、FIb与老年人颈动脉斑块相关，脉压可能是老年人发生颈动脉斑块的独立危险因素。%ObjectiveTo investigate the risk factors of carotid artery plaque in older patients through detecting the relationship among, fibrinogen,pulse pressure,carotid artery plaque and cardiovascular disease.Methods A total of 124 older patients with atherosclerotic disease were retrospectively analysised according to the status of carotid artery plaque by color doppler ultrasound.The level of fasting blood sugar,serum total cholesterol,high/low density lipoprotein,cholesterin,contractinogen,pulse pressure and glomerular filtration rate were explored in74 patients with carotid artery plaque and 50 patients without carotid artery plaque.ResultsThe level of pulse pressure,fasting plasma glucose,high-sensitivity C-reactive protein,fibrinogen was significantly higher in patients with carotid artery plaque than that in patients without carotid artery plaque(P=0.007, 0.001,0.04,<0.001).There was no significant difference on glomerular filtration rate between the two groups.After logistic regression analysis,we also found a linear correlation between pulse pressure and carotid atherosclerotic plaque(P=0.05).ConclusionsThe level of pulse pressure,fasting plasma glucose,high-sensitivity C-reactive protein and fibrinogenwere associated with carotid atherosclerotic plaque in older patients.The level of pulse pressure may be the independent risk factors for carotid atherosclerotic plaque.
