呼吸功能训练器的应用对肺癌合并轻中度慢性阻塞性肺病患者术后的影响

摘要

目的 探讨呼吸功能训练器的应用对肺癌合并轻中度慢性阻塞性肺病(COPD)患者术后恢复的影响.方法 选取108例行肺叶切除术的肺癌患者,随机分为对照组及观察组,各54例,两组患者均在术前7d及术后7天给予常规围手术期干预及护理康复指导,观察组患者在上述干预基础上采用呼吸功能训练器进行呼吸功能锻炼.于入院时、术后第7天及术后1个月分别评估患者的FVC?%、FEV1%、MVV,并采用6分钟步行距离(6-MWD)和Borg疲劳量表评定两组患者运动功能情况,同时记录两组患者术后肺部并发症发生情况.结果 观察组患者术后7d的FEV1%、MVV及术后1个月的MVV高于对照组,差异有统计学意义(P<0.05);观察组6-MWD术后第7天及术后1个月时较对照组显著增加,且在术后第7天组间差异有统计学意义(P=0.003),而Borg评分在术后第7天及1个月时均较对照组明显降低,术后第7天组间差异有统计学意义(P=0.026).两组患者术后肺部并发症发生率差异无统计学意义.结论 使用呼吸功能训练器进行2周呼吸功能训练能加快患者术后肺功能和运动功能的恢复.%Objective To?investigate?the?effect?of?respiratory?function?trainer?on?postoperative?recovery?in?lung?cancer?patients?with?mild?to?moderate?chronic?obstructive?pulmonary?disease(COPD).?Methods A?prospective?randomized?controlled?trial?with?a?total?of?108?lung?cancer?patients?with?COPD?was?conducted.?2?groups?were?both?treated?with?conventional?nursing?care?and?rehabilitation?guidance?for?two?weeks(7?days?before?and?7?days?after?the?operation).?In?addition,the?intervention?group?underwent?respiratory?function?training?with?respiratory?function?trainer?in?these?two?weeks.?The?FVC%,FEV1%,MVV,the?6-min?walking?distance(6-MWD)and?Borg?scale?were?analyzed,on?admission,7?days?and?one?month?after?surgery,as?well?as?the?incidence?of?postoperative?pulmonary?complications(PPCs).?Results In?intervention?group,the?FEV1%?and?MVV,7?days?after?operation,and?MVV,one?month?after?operation,was?significantly?higher?than?the?control?group,the?differences?were?statistically?significant (P<0.05).?6-MWD,7?days?after?the?operation,was?also?significantly?higher?than?that?in?the?control?group,and?the?results?were?significantly?different (P=0.003).?Borg?scale,7?days?after?the?operation,was?significantly?lower?than?that?in?the?control?group(P=0.026).?There?was?no?significant?difference?in?the?incidence?of?pulmonary?complications?after?operation?in?the?two?groups.?Conclusion 2?weeks'?respiratory?training?with?a?respiratory?function?trainer?accelerates?the?recovery?of?lung?function?and?motor?function?after?operation.
