This issue of the Journal of Chemical Health and Safety (November/December) is published 25 years after two of the worst disasters in the history of the chemical and process industries - the Mexico City LPG fire and explosion, and the Bhopal, India toxic gas release. I suggest that all of us who work in chemical and processing sciences and technologies take some time at the end of this year to remember those incidents, to discuss them with colleagues in safety meetings or other appropriate venues, and to re-dedicate ourselves to doing everything that we can to make sure that incidents such as these never happen again.
