Thunderclap headache is an acute and severe headachethat attains its maximum intensity within 1 hour ofonset.1 The differential diagnosis includes but is notlimited to subarachnoid hemorrhage (SAH), cerebralvenous sinus thrombosis, cervical artery dissection, andpituitary apoplexy.2–4 Current clinical practice andmajor print and online emergency medicine textbookssuggest that a negative computed tomographic (CT)head scan combined with a negative lumbar puncture(LP) result are adequate to rule out SAH in patients.
