Refinement of Vascular Access Port Placement in Nonhuman Primates: Complication Rates and Outcomes

Chronicvascularaccessisoftenneededinexperimentalanimalstudies,andvascularaccessports(VAP)havebeenproposedasanalternativetoconventionalvenipuncture.WepreviouslyreportedonVAPimplantationbyusingfemoralvenouscutdown(FVC)andtunneling.InanattempttodecreasethemoderatecomplicationsassociatedwiththeFVCmethod,wedevelopedthesingle-incision,peripheral-insertion(SIPI)method.Inaretrospectiveevaluation,92FVCprocedureswerecomparedwith113SIPIproceduresincynomolgusandrhesusmacaquesandbaboonswithasmuchas2.5yoffollow-up.TherateofcomplicationswassignificantlylowerfortheSIPImethodthanfortheFVCmethod(19.4%versus33.7%),particularlyinregardtoinfectiouscomplications(8.0%versus27.3%,respectively).Inaddition,VAPpatencyforbloodsamplingandfluidinfusionwassignificantlybetterfortheSIPImethodthanfortheFVCmethod,with1-ypatencyrateof83%and46%,respectively,and2-ypatencyrateof74%and36%,respectively.AdditionaladvantagesoftheSIPImethodincludethesimplifiedimplantationoftheVAPandaccessinthehomecagewithoutanysedationorrestraintafterappropriatetrainingofanimalstocooperate.WeconcludethattheSIPImethodpresentsanopportunityforrefinementandissuperiortotheFVCmethodforchronicvascularaccess.
