To the Editor: During the pastdecade, nosocomial infections due tomultidrug-resistant Acinetobacterbaumannii have been described withincreasing frequency, mostly in inten-sive care units (ICUs), resulting intherapeutic difficulties (1). The mainmechanism for resistance to extend-ed-spectrum cephalosporins in A.baumannii is attributed to the overex-pression of chromosome-encodedcephalosporinases or to plasmid-encoded Ambler class A, B, and D β-lactamases (2). A. baumannii that pro-duce PER-1 extended-spectrum β-lac-tamase (ESBL) are rarely isolatedoutside Turkey and remain suscepti-ble to carbapenems (3). Here wedescribe what we believe is the firstESBL-producing A. baumannii iso-late resistant to carbapenems and thefirst characterization of a PER-1 A.baumannii isolate from Russia, fur-ther supporting the emergence anddissemination of PER-1 A. baumanniistrains in eastern Europe and outsideTurkey (3,4).
