In 1977 two 747s collide in Tenerife killing 583 people. Aircraft crashes onto M1 Motorway near Kegworth killing 39, seriously injuring 70. Aircraft crashes make headline news because of the sheer numbers of people killed or injured in one incident. However, in comparison to other modes of transport, the overall safety record for air travel remains extremely good. Less than 14,000 people perished in US airline accidents between the start of public aviation and 2000 compared to almost three times that number who die each year in road accidents. Technical advancements have made air travel much safer but of the few aircraft accidents that occur, more than 80 per cent are caused by human error, and often by pilots with enormous experience. Main causes of these errors include: 1. poor communication; 2. unresolved ambiguity; 3. insufficient assertiveness; 4. reduced situational awareness; and 5. time pressure.
