The psycho-oncological literature has only recently addressed the situation of older patients in an explicit manner. The topic has, however, been implicitly addressed because of the mere frequency of cancer in older persons. Concrete information about psychosocial impairments in older cancer patients is lacking as is a systematic assessment of age-specific psycho-oncological diagnosis and treatment. Based on recent psycho-oncological publications the present paper addresses the psychosocial situation of older cancer patients as compared to healthy peers, younger patients and those with different cancer forms. Approaches to age-related psycho-oncological diagnostic procedures and treatment options are described and critically discussed in terms of future research needs. Analyses suggest that at least one quarter of patients suffer from impairments of wellbeing and function even though interindividual differences jeopardize an age-related classification of psychosocial problems. Diagnostic methods to assess mental health and quality of life are also available in older patients but are used for research purposes more than in clinical practice. Despite the wealth of psycho-oncological interventions only few options are tailored to older patients and an evaluation of psycho-oncological care in the elderly is just beginning.
