It is difficult to determine the optimal dose when 3 antihypertensive drugs are combined in a single tablet. Moreover, the availability of multiple dose strengths with poorly differentiated packaging creates a risk of prescribing and dispensing errors. The first-choice drug for otherwise healthy hypertensive patients is a thiazide diuretic such as chlortalidone, or else hydrochlorothiazide (1). When well-conducted thiazide therapy is not sufficiently effective, single-agent treatment with amlodipine, a dihydropyridine class calcium channel blocker, and valsartan, an angiotensin II receptor blocker, have both been shown to prevent car-diovascular events (1). Two-drug combinations are third-line options but their potential for adverse effects and drug-drug interactions must be taken into account (1,2).
