摘要

Purpose:To determine whether a written educational resource used by nurses at discharge could improve patient knowledge of cardiovascular medications at home.Summary:Understanding and taking multiple medications can become problematic for elderly patients who have been discharged. Nurses can teach patients self-medication skills on all the actions such as getting the prescription, taking the medicines on time and observing symptoms of problems. Increasing patient knowledge and understanding of their medications, can lead to increased adherence. It is proposed that nurses can implement a verbal and written medication educational program for patients being discharged from the hospital which have shown that patients can be adherent with the medications initiated during hospitalization. Therefore, an advanced practice nursing intervention was planned to increase patient outcomes by introducing a structured verbal and written medication educational program. The purpose of the research was to examine the impact of a written educational resource used by nurses at discharge on patient knowledge of cardiovascular medications. (27 refs.)
