Valproate is an effective treatment for bipolar disorder and epilepsy. Valproate has also been frequently associated with self-limited asymptomatic transami-nase elevations or hyperammonemia or both. Life-threatening cases of liver failure, though, are much less common (1-3 per 100,000 person-years). We report a case of fulminant hepatic failure associated with valproate reintroduction in a patient with previous valproate exposure.
