摘要

Multiple pathologies occur in the cavernous sinus. Despite the anatomic complexities of this region, many of these pathologies can be addressed surgically. Such surgical approaches require significant preoperative evaluation and preparation, Two major surgical approaches to the cavernous sinus are discussed in this article: the cranioorbitozygomatic (COZ) and the extended middle fossa approaches. The COZ approach can be tailored for superior entry into the cavernous sinus and intra- or extradural entry to the lateral cavernous sinus. Lesions in the posterior or lateral cavernous sinus can also be approached through the extended middle fossa approach. The steps involved in each of these approaches are illustrated and discussed.
