An infant with all of the cardinal features of birth asphyxia as specified by the American College of Obstetrics and Gynecology went on to develop severe spastic quadriplegic cerebral palsy. However, placental pathology revealed 2 significant lesions indicating a period of subacute vascular injury and severe hypoxia extending 6 to 12 hours or more prior to the delivery. The first, meconium-associated vascular necrosis, is a fetal large vessel vasculopathy related to apoptosis of vascular smooth muscle cells secondary to the toxic effects of prolonged exposure to high concentrations of meconium. The second, markedly increased circulating fetal nucleated red blood cells, is an adaptive response by fetal hematopoietic tissues to reduced circulating fetal oxygen tension.
