Dear Sir, A 68-year-old man suffered from unexplained pain and slight swelling of the right index finger. He was generally healthy, except for the use of aspirin and antihypertensive drugs. Trigger finger was diagnosed and several intra-tenosynovial corticosteroid injections were given. However, this treatment failed to improve his symptoms and the redness and swelling increased. He presented to us 5 months after developing the condition. On examination, the entire right index finger appeared swollen, red, and tender. Laboratory examination revealed a normal white blood count of 4450/mul and C-reactive protein <0.1 mg/dl. However, cephem antibiotics were prescribed on suspicion of suppurative tenosynovitis. The swelling and redness worsened and extended to the palmar region, and surgical treatment was therefore scheduled.
