Clinical characterization of dermatornyositis associated with mechanic's hands

"Mechanic's hands" was first described by Stahl et al. as a cutaneous manifestation of inflammatory myositis including dermatomyositis. This skin sign has been correlated with au-toantibodies against aminoacyl tRNA synthetases (ARS), including Jo-1, that are myositis-specific markers. Matsushita ef al. precisely studied dermatomyositis patients with anti-ARS antibodies (antisynthetase syndrome), comparing to those patients without them, and characterized that the former subgroup had higher incidence of interstitial pneumonia (IP), arthritis, fever and Raynaud's phenomenon, but not associated with internal malignancy, and tended to express mechanic's hands as a skin sign. Furthermore, Bachmeyer ef al. studied mechanic's hands in seven patients with antisynthetase syndrome indicating this skin sign for respiratory and systemic involvement. Being compatible with these observations, most cases of dermatomyositis with mechanic's hands have been reported with IP but not with internal malignancy, while some sporadic dermatomyositis patients with mechanic's hands were associated with malignancy.
