Reliable surrogate endpoints could potentiate and foster the process of clinical research in solid tumours that aims to cure or extend survival. A surrogate endpoint is perfect if it is causally related to survival (ie, the effect of treatment on the surrogate explains the whole effect on survival or a large part of it), and if it is easy to assess and can be reached quickly, much earlier than the ultimate goal. Methods to validate the quality of a surrogate endpoint are complex, need a large amount of data, and, even when used in the best way, caution is needed in the interpretation of results.
