Intraoperative lymphatic mapping and sentinel lymphadenectomy have become common practice for breast cancer, melanoma, and other malignancies. Triphenylmethane-based dyes (Patent Blue V, Laboratoire Guerbet, Aulney-Sous-Bois, France) or its derivative, isosulfan blue, are commonly used for this purpose. The reported incidence of allergic reactionsto these dyes is 1% to 2%, ranging from mild local reactions to anaphylactic shock.We report two melanoma patients who developed allergic reactions with "blue hives" after intradermal triphenylmethane-based dye injection, one of them with full-scale anaphylactic shock. Most reports, so far, describe such reactions after the injection of large amounts of dye into the breast parenchyma.
