We report a case of broncho-mediastinal fistula in a 73-year-old man who presented with fever and hoarseness. Chest computed tomography showed a 35 X 45-mm mass in the left hilum and an adjacent cavity measuring 60 x 25 mm, He was diagnosed as having squamous cell carcinoma of the lung on the basis of sputum cytology. Flexible bronchoscopy revealed a broncho-mediastinal fistula between the left lingular bronchus and the mediastinal cavity. The patient died of massive hemoptysis 1 month later, apparently as a result of rupture of the pulmonary artery.
