Usefulness of three-dimensional ultrasonography in the prenatal evaluation of acromelic deviations.

The increasing use of three-dimensional sonography (3D US) in clinical practice has focused attention on the role that this kind of imaging may play in the evaluation of fetal abnormalities. We would like to emphasize its benefit in the evaluation of fetal limb extremity malformations previously suspected by two-dimensional sonography (2D US). As far as the upper limbs are concerned, our previous report concerning a case of isolated bilateral radial dysplasia highlighted that a persistent abnormal hand posture and a fixed flexion of the wrist, typical of this condition, was better demonstrated by 3D US. However, this deformity is uncommon and few physicians will have the opportunity to observe it in their practice. In contrast, the finding of a foot fixed in adduction, supination and varus (clubfoot), is more frequent (1-3:1000), and all sonographers may come across this birth defect in their daily practice and may therefore benefit from the use of this new technology. This malformation, which is bilateral in 50% of cases, more frequently affects boys and may be isolated or associated with genetic syndromes or other major structural abnormalities including cleft palate, congenital heart disease, inguinal hernia, or dislocation of the hip. In addition to genetic factors, there are extrinsic causes including severe oligohydramnios or amniotic bands. Although this malformation is most often isolated, physicians should search for other associated findings. Isolated idiopathic clubfoot may be classified based on the age of prenatal detection. It ranges in severity from mild forms, which often do not require surgical treatment, moderately severe forms, and severe forms.
